One complete Doctor Sebas guide to USMLE study, ECFMG, OET, clinical experience, research, applications, interviews, Step 3 and residency planning.

Turn this guide into a connected training plan

Read a chapter, practice the related skill, review what you missed and return to your route. These activities complement your exam-specific preparation and official application resources. Choose the tools that fit your current stage. Each tool keeps its own progress.

CardioLab: Practice ECG interpretation and murmur recognition. Explain what you noticed before revealing an answer. Next: Then explain the same finding aloud, as if presenting to your team. Practice with CardioLab →

EndoLab: Choose an endocrine topic, read the explanation, then practice its questions. Compare your reasoning with the explanation after answering. Next: Return to your question bank and look for the same reasoning pattern. Practice with EndoLab →

ChartSim EMR: Work through a simulated patient chart. Justify your orders and next steps, then review the case feedback. Use it alongside your exam-specific preparation. Next: Write the assessment and plan in Note Lab. Practice with ChartSim EMR →

Note Lab: Practice a focused H&P, SOAP note, consult or discharge summary. Make the problem representation, assessment and plan easy for another clinician to follow. Next: Give a concise verbal handoff without reading the entire note. Practice with Note Lab →

Doctor Sebas Languages: Practice listening, speaking and patient-facing language. Use this to build communication habits alongside the official OET preparation materials. Next: Try a short patient explanation and check whether it is clear and jargon-free. Practice with Doctor Sebas Languages →

Personal Statement Lab: Choose your specialty and build a statement from real moments. Explore patient stories, teamwork, leadership, volunteering or a personal turning point. You can also paste a draft and review what is missing. Next: Use the same facts to prepare interview examples. Final application prose should be in English. Practice with Personal Statement Lab →

Practice Filling Out ERAS: Open the Lab and select ERAS Practice. Organize education, experiences, meaningful reflections, scholarly work and document assignments. Check character counters and download a backup before copying to MyERAS. Next: Compare the practice draft with the current AAMC worksheet and your actual MyERAS portal. This tool does not submit applications. Practice with Practice Filling Out ERAS →

Letters of Recommendation Lab: Open Letters in the Lab. Prepare a request and a factual achievement sheet for a supervisor who knows your work. Include examples they personally observed. Next: Confirm the request, deadline and official submission instructions with the writer. Practice with Letters of Recommendation Lab →

Interview Prep: Choose an interview, practice timed answers and review the feedback. Use your actual ERAS experiences and personal statement so your spoken answers tell a consistent story. Next: Practice “why this specialty,” teamwork and an example of growth. Use outlines, not memorized scripts. Practice with Interview Prep →

Use the interactive guide to choose your next activity. Its training checklist and notes save on your device; you can download your practice plan. Open the USMLE guide →

1. Start here: eligibility, ECFMG, accreditation and registration

Understanding the USMLE

The United States Medical Licensing Examination (USMLE) is a three-Step medical licensing examination sequence used on the U.S. physician licensure pathway. For an IMG it also does a second job: Step 1 and Step 2 CK are part of the requirements for ECFMG Certification, which you need to enter a U.S. residency.

Step 1 Basic science applied to medicine 1 day · 14 blocks · Pass/Fail

Step 2 CK Clinical knowledge and reasoning 1 day · 16 blocks · 3-digit score , numerically scored

Step 3 Independent practice and management 2 days · MCQ + CCS cases · numerically scored

Who does what

Organization: USMLE program (FSMB + NBME) · What it does for you: Owns the exams, the rules and the Bulletin of Information.

Organization: FSMB · What it does for you: Since January 12, 2026, handles USMLE registration, permits, scores and transcripts for all IMGs and for all three Steps.

Organization: Intealth / ECFMG · What it does for you: Verifies your medical school and diploma, runs the Pathways, and grants ECFMG Certification. Your account is MyIntealth.

Organization: NBME · What it does for you: Writes the exams and sells the official self-assessments (CBSSA, CCSSA, CCMSA).

Organization: Prometric · What it does for you: Runs the test centers where you sit the exam.

Organization: OET · What it does for you: Runs OET Medicine, the English communication test ECFMG requires.

What changed in 2026

  • IMG exam registration moved to FSMB. IMGs now apply for Step 1, Step 2 CK and Step 3 through the FSMB USMLE portal. ECFMG still decides eligibility and certification.

  • Shorter blocks. Step 1 (from May 14, 2026) and Step 2 CK (from May 7, 2026) now use 30-minute blocks of up to 20 questions, with a new interactive testing software.

  • Same fees for 2026 and 2027: $695 for Step 1, $695 for Step 2 CK, $955 for Step 3. Testing outside the U.S. and Canada adds an international surcharge.

The order most IMGs follow

You can take Step 1 and Step 2 CK in either order once you are eligible. Most IMGs go Step 1 → Step 2 CK → OET → Pathway → ECFMG Certification → ERAS and the Match → Step 3, often during residency. Your situation (graduation status, license, Match year) can change that order. The next stops walk through each piece.

Dr. Sebas tip: Step 1 is Pass/Fail, so programs look harder at your Step 2 CK score. Treat Step 1 as your foundation and Step 2 CK as your number.

Am I eligible? Your school and ECFMG

Before you study a single page, make sure your medical school and your documents will be accepted. This is the step people skip, and it is the one that can block everything later.

Check these in order

  1. Find your school in the World Directory of Medical Schools (WDOMS). Open the ECFMG tab and read the Sponsor Notes. Being listed is not enough: the notes say which graduation years are eligible.

  2. Create your MyIntealth account and complete the application for ECFMG Certification. Use the name and identity details required by MyIntealth and your exam registration organization; resolve inconsistencies before testing.

  3. Verify your credentials. Intealth contacts your school to confirm your diploma and transcript. Ask your school who handles this, because a slow response delays your exams.

  4. Students: you can test while enrolled if your school confirms you are an officially enrolled student. Graduates: your diploma must be verified.

  5. Then apply for the exam in the FSMB USMLE portal (next stop).

Time limits you must track yourself

Rule: Seven-year window · What it means: For ECFMG, your exam requirements must be met within seven years starting from the first exam you pass. Your Pathway must also be accepted inside that window. ECFMG does not send reminders.

Rule: Attempt limits · What it means: No more than three attempts at the same Step in 12 months. A fourth attempt needs at least 12 months since your first attempt and 6 months since your most recent one. If you have attempted a Step four or more times without passing (incomplete attempts count), you can no longer apply for any Step.

Rule: Retaking a passed Step · What it means: You cannot retake a Step you passed to improve your score. Limited exceptions can apply under the USMLE policy for time limits imposed by recognized authorities. Check the official rule for your situation.

Before you pay anyone: no agency can make you eligible, speed up verification, or guarantee a result. Eligibility comes only from WDOMS, Intealth and the USMLE rules.

Dr. Sebas tip: Every attempt stays on your transcript. Book the exam when your practice data says you are ready, not when the calendar says so.

Your school’s accreditation and the WFME

The World Federation for Medical Education (WFME) does not accredit medical schools. It reviews the agencies that accredit them, and recognizes the ones that meet international standards. Intealth (ECFMG) uses that recognition in two places.

1. The Recognized Accreditation Policy

What it says: When · A school meets the policy if it is accredited by an agency recognized by the WFME or by the U.S. NCFMEA (National Committee on Foreign Medical Education and Accreditation), and it meets ECFMG’s medical school requirements.: Introduced by ECFMG in 2010 (first called the “2023 requirement”). Implementation began in November 2024.

What it says: Does it block the USMLE? · A school meets the policy if it is accredited by an agency recognized by the WFME or by the U.S. NCFMEA (National Committee on Foreign Medical Education and Accreditation), and it meets ECFMG’s medical school requirements.: No. Intealth states that the policy does not affect eligibility to take the USMLE.

What it says: Does it block ECFMG Certification? · A school meets the policy if it is accredited by an agency recognized by the WFME or by the U.S. NCFMEA (National Committee on Foreign Medical Education and Accreditation), and it meets ECFMG’s medical school requirements.: No. Intealth states it has no impact on eligibility for ECFMG Certification. You can apply even if your school does not meet it.

What it says: How to check · A school meets the policy if it is accredited by an agency recognized by the WFME or by the U.S. NCFMEA (National Committee on Foreign Medical Education and Accreditation), and it meets ECFMG’s medical school requirements.: Find your school in WDOMS, open the Sponsor Notes tab, and look for the accreditation notation below the ECFMG Sponsor Note.

Why care if it does not block you? Intealth says the policy will keep evolving, and accreditation is how residency programs and state boards judge where you trained. It also decides which Pathway you can use.

2. Pathways 3 and 4

Pathway 3 is for students and recent graduates of schools accredited by a WFME-recognized agency. Pathway 4 is for schools accredited by an agency with an NCFMEA comparability decision. Also review Pathway 5 for eligible joint-degree schools. Pathway 6 applies when Pathways 1–5 do not apply, or when required because of a prior Step 2 CS failure. The current ECFMG eligibility details determine your route.

Check yourself in four steps

  1. Search your school in WDOMS and read the ECFMG Sponsor Notes for your graduation year.

  2. Look for the Recognized Accreditation Policy notation under the Sponsor Note.

  3. Find your country in the World atlas and see which agency is WFME-recognized there and until when.

  4. Ask your dean’s office which agency accredits your school and when its accreditation was last renewed.

All WFME-recognized agencies by country

Some agencies work in many countries (for example CAAM-HP and ACCM in the Caribbean, IAAR and ECAQA in Central Asia, TEPDAD in the Middle East). Dates show the end of the recognition period listed by WFME.

Agencies currently applying for WFME recognition

Croatia (ASHE), Bangladesh (BMEAC), selected French-speaking countries such as Cameroon, Morocco and Tunisia (CIDMEF), Greece (HAHE), Nepal (MEC), Bulgaria (NEAA), Uzbekistan (NQAAE), Vietnam (VNU-HCM Center for Education Accreditation) and Yemen (CAQA, process paused).

Dr. Sebas tip: Ask your school for its accreditation in writing early. It takes one email now, and saves you weeks when you fill out your Pathway application.

Apply, pay and schedule your exam

From application to a seat at Prometric

  1. Create your FSMB account and apply for the Step in the USMLE portal. Pay the fee and choose a three-month eligibility period.

  2. Wait for ECFMG to confirm eligibility. Once approved, your scheduling permit appears in the portal. Download it.

  3. Check the name on the permit against your passport, letter by letter. If it does not match, fix it before exam day.

  4. Book at Prometric with the number on your permit. Seats go first come, first served, and you can book up to about six months ahead.

  5. Save everything: permit, confirmation number, receipts and dates.

Build a current exam budget

Check the FSMB fee schedule and your checkout total for the Step application, international delivery charges, taxes, eligibility changes and rescheduling. Record the source and date; do not assume a figure from a screenshot applies to your testing region.

Application fees are non-refundable and non-transferable. If you do not test in your eligibility period, you have to apply and pay again.

Test accommodations

If you have a documented disability, request accommodations through the USMLE accommodations process before you schedule. Approval takes time.

Dr. Sebas tip: Pick your eligibility period backwards from the date you want to test, and leave room to move the date once. Don’t buy a nonrefundable flight until your appointment is confirmed.

Check your understanding

Since January 2026, where does an IMG apply for Step 1?

The FSMB USMLE portal — FSMB now manages USMLE registration for all IMGs. ECFMG still decides eligibility and grants certification.

Which Step is reported only as Pass/Fail?

Step 1 — Step 1 has been Pass/Fail since January 26, 2022. Step 2 CK and Step 3 keep a three-digit score.

What is the minimum passing score for Step 2 CK?

218 — 218, for exams taken on or after July 1, 2025.

Your school appears in WDOMS. Are you automatically eligible?

No, read the ECFMG Sponsor Notes for your graduation year — The Sponsor Notes say which students and graduation years ECFMG accepts.

How many times can you take the same Step in 12 months?

Three — Three attempts within 12 months, with extra waiting rules for a fourth attempt.

Who does the WFME recognize?

Accrediting agencies — WFME recognizes accrediting agencies. Agencies accredit schools.

Your school does not meet the Recognized Accreditation Policy. Can you still take the USMLE?

Yes — Intealth states the policy does not affect USMLE eligibility or ECFMG Certification eligibility.

Which Pathway is for schools accredited by a WFME-recognized agency?

Pathway 3 — Pathway 3. Pathway 4 is the NCFMEA equivalent.

How long is a USMLE eligibility period?

Three months — Check the current application instructions, eligibility period and fee schedule before booking or making a change.

Which document do you need to book at Prometric?

Your scheduling permit — The scheduling permit, downloaded from the portal once eligibility is confirmed.

2. Step 1 and Step 2 CK: formats and a study system

Step 1: the foundation

Step 1 tests whether you can apply basic science to clinical situations: mechanisms, pathophysiology, pharmacology, microbiology, behavior, biostatistics and ethics. Memorizing isolated lists is not enough. You have to explain why a finding happens and what it changes.

Exam day format (from May 14, 2026)

The blocks are shorter than before. Practice switching blocks often and deciding when to use your breaks.

A study system that works

Piece: One main question bank · How to use it: Tutor mode by system at first, then timed random blocks. · How you know it’s working: Accuracy goes up and the same mistakes stop repeating.

Piece: Error log · How to use it: Write why you missed it (knowledge, reasoning, reading, timing, language) and one short rule. · How you know it’s working: You have short rules you review on a schedule.

Piece: Targeted content · How to use it: Videos or text only for the weak areas your questions show you. · How you know it’s working: Less passive reading.

Piece: Active recall · How to use it: Explain without looking. Use flashcards selectively. · How you know it’s working: You remember it on new questions.

Piece: NBME CBSSA · How to use it: The official Step 1 self-assessment, every few weeks. · How you know it’s working: A clear trend and named weak areas.

How long should you study?

Length: 8 weeks · Hours / week: 45–60 h · Shape: Baseline, intense question bank, error review, CBSSA in weeks 1, 4 and 7–8.

Length: 12 weeks · Hours / week: 25–40 h · Shape: Targeted foundations, questions by system then mixed, CBSSA every 3–4 weeks.

Length: 16 weeks · Hours / week: 20–35 h · Shape: Rebuild weak bases, progressive questions, mixed final phase.

Length: 24 weeks · Hours / week: 15–25 h · Shape: Basic science plus systems, gradual questions, dedicated final period.

Build your own week-by-week version in Your study toolkit.

How to work a question

  1. Read the last line first. What mechanism or relationship is being tested?

  2. Separate the key data from the noise.

  3. Before reading the explanation, say why you chose your answer.

  4. Label the error and write one short rule. Don’t copy the whole explanation.

  5. Test the rule on a different question 3–7 days later.

The last 14 days

  • Do at least one full simulation with breaks.

  • Practice the official testing software and sample questions on usmle.org.

  • Stop adding new resources.

  • Check your ID, permit, center and transport.

  • Protect sleep and meals.

If you are retaking

More hours alone won’t fix it. Look at your score report, your error types, the exam-day logistics, your health and how you used your resources. Then build a different plan.

Dr. Sebas tip: There is no universal CBSSA score that means ‘book it’. Look at the trend across several assessments, your timing, and whether you can explain your mistakes.

Step 2 CK: your score

Step 2 CK tests clinical knowledge for supervised patient care: diagnosis, choosing tests, treatment, prevention, communication, safety and professionalism. Read every question as a clinical decision. Because Step 1 is Pass/Fail, this score carries a lot of weight in your residency application.

Exam day format (from May 7, 2026)

Nine hours is long. Plan your breaks so you are not exhausted for the last blocks.

A reasoning framework for every question

  1. Is the patient stable or unstable? How urgent is it?

  2. What is the task: diagnosis, test, treatment, prevention, prognosis, ethics or safety?

  3. Tell apart best initial test, most accurate test and next best step.

  4. Factor in age, pregnancy, other conditions and timing.

  5. Remove options that are too early, too late, too invasive or irrelevant.

  6. Include communication and what the patient wants.

High-yield areas people underestimate

  • Screening and prevention (follow the USPSTF, CDC, ACOG and AAP guidelines)

  • Ethics, consent, capacity and confidentiality

  • Quality improvement and patient safety

  • Communication and disclosing medical errors

  • Obstetric and pediatric emergencies

  • First steps for an unstable patient

  • Biostatistics and reading studies

Plans and self-assessments

Length: 8 weeks · Best for: Full-time dedicated study · Shape: Intense question bank, selected CMS forms, CCSSA, timed mixed blocks, close gaps.

Length: 12 weeks · Best for: Working or on rotations · Shape: Questions by system, error log, mixed phase, CCSSA monthly.

Length: 16 weeks · Best for: Uneven clinical base · Shape: Targeted clinical review, progressive questions, CMS, simulations.

CCSSA is the NBME self-assessment for Step 2 CK. Clinical Science Mastery Series (CMS) forms let you test one discipline at a time. After each one, write down the date, your weak areas and one concrete action.

Timing it with ERAS

For the 2027 Match, programs began reviewing applications on September 23, 2026. Scores usually take about four weeks to report. If your Step 2 CK score is part of your application, test early enough for it to arrive.

Dr. Sebas tip: Set your target score from your specialty and the programs you want, then adjust it with real data from your self-assessments. Other people’s scores on social media are not your plan.

Current block structure

For examinations on or after May 14, 2026, Step 1 uses fourteen 30-minute blocks within an eight-hour session. Step 2 CK uses sixteen 30-minute blocks within a nine-hour session for examinations on or after May 7, 2026. Each block contains up to 20 questions. Use the official practice interface for your exam date.

Practice the system you just studied

After reviewing cardiovascular reasoning, use CardioLab for additional ECG, murmur and cardiovascular practice. After reviewing endocrine topics, use EndoLab to connect concepts with visual study and questions. Return to your error log afterward and record what you still need to clarify.

Check your understanding

How long is each Step 1 block in the 2026 format?

30 minutes — 14 blocks of 30 minutes, up to 20 questions each.

What is the official NBME self-assessment for Step 1?

CBSSA — CBSSA (Comprehensive Basic Science Self-Assessment). CCSSA is for Step 2 CK and CCMSA for Step 3.

What goes in your error log?

The reason you missed it and one short rule — Short rules you can re-test are what make the log useful.

How many 30-minute blocks are in Step 2 CK?

16 — 16 blocks in a 9-hour day.

What should you decide first in a clinical vignette?

Whether the patient is stable — Stability and urgency come first. They change what the next step can be.

3. OET Medicine and the ECFMG Pathways

OET Medicine and the Pathways

The examination components of ECFMG Certification include medical science (Step 1 and Step 2 CK), clinical skills (a Pathway) and communication skills (OET Medicine). All applicants using a Pathway must meet its OET Medicine requirement, regardless of native language or citizenship. Applicants who already satisfied the clinical and communication requirements through a former accepted examination follow their applicable ECFMG rules.

The four OET Medicine subtests

Subtest: Listening · Time: ≈ 40–45 min · What you do: Consultation extracts, workplace audio and presentations · ECFMG minimum: 350

Subtest: Reading · Time: 60 min · What you do: Quick scanning and detailed healthcare texts · ECFMG minimum: 350

Subtest: Writing · Time: 45 min · What you do: One letter (usually referral) from case notes · ECFMG minimum: 300

Subtest: Speaking · Time: ≈ 20 min · What you do: Two role-plays with a patient played by an interlocutor · ECFMG minimum: 350

  • All four minimums in one sitting. If you miss one, you retake all four.

  • Scores must be from a test taken on or after January 1, 2025.

  • No limit on retakes, as long as there are dates available.

  • For results released on or after May 14, 2026, give OET your MyIntealth ID (not your USMLE ID) when you book, so results reach ECFMG.

  • Results usually come about 10 business days after the test, and take about 3 more business days to show in your Pathways application.

  • Formats: paper or computer at a venue. OET@Home counts only if your country has no venue. Check OET’s booking portal for the current total for your test format and location.

The six Pathways (clinical skills)

Pathway: 1 · For whom: Currently or recently licensed to practice without supervision in another country. If you qualify, you must use Pathway 1.

Pathway: 2 · For whom: Passed a licensing OSCE given by your school and required by your country’s regulator.

Pathway: 3 · For whom: Student or recent graduate of a school accredited by a WFME-recognized agency.

Pathway: 4 · For whom: Student or recent graduate of a school accredited by an agency with an NCFMEA comparability decision.

Pathway: 5 · For whom: School that grants a joint degree with a U.S. LCME-accredited school.

Pathway: 6 · For whom: Everyone else. Licensed physicians evaluate your clinical skills with the ECFMG Mini-CEX for Pathway 6. Required if you ever failed Step 2 CS.

Deadlines for the 2027 Match

  • Take OET Medicine by the last test date in December 2026.

  • Submit your Pathway application, OET scores and documents by January 31, 2027.

  • All exam requirements must be met by the NRMP Rank Order List deadline.

A certificate based on a Pathway can expire with that Pathway unless it is revalidated, so read the expiration rules for your cycle.

Dr. Sebas tip: Treat OET like a clinical exam, not an English class. Practice speaking role-plays out loud with a partner and get your letters corrected. Our Project IMG Discord is a good place to find a study buddy.

Official sources: ECFMG OET rules · 2027 Pathways · OET

A weekly OET practice routine

Choose a recurring appointment for each skill rather than practicing only the section you enjoy. Record one speaking role-play, identify one communication habit to improve, rewrite one referral letter after feedback, and review the reason for each listening or reading error. Track your next action, not just the practice score.

Check your understanding

What OET Writing score does ECFMG require?

300 — 300 on Writing, and 350 on Listening, Reading and Speaking, all in one sitting.

You passed three OET subtests and missed Speaking. What now?

Retake all four subtests — ECFMG requires all four minimums in a single administration.

You are licensed in your country. Which Pathway applies?

Pathway 1 — If you qualify for Pathway 1 you must use it, unless you failed Step 2 CS.

4. Exam day, score reports and retakes

Exam day at Prometric

What happens when you arrive

  1. Arrive at least 30 minutes early with your scheduling permit (paper or electronic) and a valid, unexpired government photo ID with your signature, usually your passport. The name must match the permit exactly. If you are more than 30 minutes late you will not be admitted and must pay to reschedule.

  2. Check-in: Prometric verifies your ID, takes your photo and runs a security check. Everything goes in a locker. Phones and devices must be turned off, not just silenced.

  3. In the room: staff give you laminated writing surfaces and markers, and your station has headphones. You may bring only your locker key and ID. Soft-foam earplugs without strings are an approved item.

  4. Breaks: taken only between blocks. You sign out and back in each time. If you use more than your break time, the extra is taken from your testing time.

  5. A technical problem? Tell the test center staff right away, then email testadmin@nbme.org within 10 calendar days with your name, USMLE ID, exam, date and what happened. If you skip either step you may lose the right to a remedy.

Your break strategy

You get at least 55 minutes of break time for Step 1 and Step 2 CK (more if you skip the tutorial). A common plan is a short break every two or three blocks and a longer break in the middle for food. Practice the plan in your simulations.

Dr. Sebas tip: Your break time is a resource. Don’t spend it rereading notes or talking about questions. Eat, drink water, rest your eyes, go back in.

Scores, transcripts and ECFMG Certification

When results arrive

Results are usually ready within four weeks of your test date. Allow up to eight weeks before you worry. You get an email when your score report is available in the portal.

Exam: Step 1 · Reported as: Pass / Fail · Minimum to pass: Pass (standard set internally)

Exam: Step 2 CK · Reported as: 3-digit score · Minimum to pass: 218

Exam: Step 3 · Reported as: 3-digit score · Minimum to pass: 200

Transcripts for ERAS

Check the current FSMB fee schedule and the amount shown for your specific service, exam and testing region before paying. Include taxes, travel and any rescheduling costs in your budget.

If you did not pass

  • Read the performance profile: it shows your weaker areas.

  • Check the waiting rules before you reapply (Stop 2).

  • Change the method, not just the hours. Talk to a mentor before you book again.

Becoming ECFMG certified

When Step 1, Step 2 CK, OET and your Pathway are complete and your diploma is verified, ECFMG issues your certificate. You need it to start residency, and as an IMG you need it before you can apply for Step 3.

Dr. Sebas tip: A failed attempt is information, not the end of your path. Many residents had one. What programs look for is what you changed and how you did after.

Official sources: Results & scoring · FSMB transcripts

Before planning another attempt

Review your score report, preparation method and exam-day conditions. Check the current attempt limit, waiting periods and any licensing-board time limits before booking. The USMLE eligibility policy addresses incomplete attempts and restricted exceptions for retaking a previously passed Step.

Check your understanding

What can you take into the testing room?

Your locker key and ID; staff give you the writing surfaces — Everything else goes in the locker. Staff provide laminated writing surfaces and markers.

You have a technical problem during a block. What do you do?

Tell staff right away, then email USMLE within 10 days — Tell the staff on the day and email testadmin@nbme.org within 10 calendar days. Survey comments do not count.

How long do results usually take?

About four weeks — Typically within four weeks; allow up to eight.

Which organization sends your USMLE transcript to ERAS for 2026–2027?

FSMB — FSMB handles USMLE transcripts for ERAS.

5. Step 3 and CCS practice

Step 3: ready for independent practice

Step 3 is the last exam in the series. It asks whether you can manage patients without supervision. Many IMGs take it during residency. Some take it before the Match because it can help with certain visa sponsorships, but that decision depends on your situation.

Who can apply

  • An MD or equivalent degree

  • Passed Step 1 and Step 2 CK

  • ECFMG Certification (for IMGs)

  • Apply through the FSMB portal. Testing is only in the U.S. and its territories.

Passing Step 3 does not give you a license. Each state has its own extra requirements.

Two exam days

: Focus · Day 1: Applied science, pharmacology, epidemiology, ethics, biostatistics · Day 2: Diagnosis, management, prognosis, prevention, follow-up

: Prepare with · Day 1: Biostatistics, reading abstracts, applied basic science · Day 2: Clinical management and CCS practice

CCS: Computer-based Case Simulations

You manage a virtual patient in simulated time, writing orders the way you would in an electronic chart. Work every case in this order:

  1. Check stability and immediate threats.

  2. Do urgent interventions without waiting for tests when needed.

  3. Order relevant studies. Avoid ordering everything.

  4. Choose the right care setting and consults.

  5. Reassess results and response.

  6. Add prevention, counseling and follow-up.

  7. Close the case with a coherent plan.

Practice CCS-style cases with the Project IMG EMR simulator on doctorsebas.com, and use the official CCS practice software on usmle.org to learn the real interface.

Common mistakes

  • Studying Step 3 as a repeat of Step 2 CK

  • Leaving CCS until the last days

  • Ignoring pharmacology, biostatistics and ethics on Day 1

  • Ordering too much in CCS without priorities

  • Not planning around call schedules and fatigue

CCMSA is the NBME self-assessment for Step 3 (200 questions).

Dr. Sebas tip: Start CCS practice in week one, not week six. The interface is a skill of its own, and it only gets faster with reps.

Make CCS practice measurable

After a fictional case, write down the task you missed: identifying instability, prioritizing the first action, choosing a care setting, reviewing a result or arranging follow-up. Repeat the case with one improvement target. The Doctor Sebas simulator provides additional practice; use official USMLE software to learn the actual examination interface.

Practice communicating your clinical reasoning

Use Note Lab to turn a fictional case into a clear clinical note. Practice explaining your reasoning, writing a focused assessment and organizing the plan. These tools support study and rehearsal; they do not grant clinical privileges or replace supervised training.

Check your understanding

Can you take Step 3 in Canada?

No, only in the U.S. and its territories — Step 3 is only offered in the U.S. and its territories.

In a CCS case, what comes first?

Assess stability and treat immediate threats — Stabilize first. Tests and follow-up come after.

6. USCE, research, applications, interviews and the Match

From the exams to the Match

Passing the required exams is one part of eligibility. Your complete application helps programs decide whom to interview. Start preparing well before your application service opens.

Use Match data carefully

Review the current NRMP reports for your applicant group and specialty. A group match rate describes a population; it does not predict an individual applicant’s chance of matching.

Build your application calendar

Use the AAMC timeline for ERAS programs, the application service used by your specialty, the NRMP calendar and each program’s instructions. Submission, review, interview, ranking and Match Week dates are different milestones. Certification and document deadlines need their own buffer.

U.S. clinical experience (USCE)

Type: Elective · What it is: A rotation for enrolled students · Check before you commit: Active enrollment, VSLO and the hospital’s rules

Type: Hands-on / externship · What it is: Supervised clinical participation · Check before you commit: Legal scope, insurance and a written policy

Type: Observership · What it is: Observing, no clinical responsibility · Check before you commit: Don’t assume patient contact or a letter

Type: Research · What it is: Work on a project or in a group · Check before you commit: Your role, mentor, ethics approval and authorship

What goes into a residency application

  • Personal information, education and training

  • Experiences, including your most meaningful ones

  • Publications, research, volunteering

  • Exams and certifications (your USMLE transcript from FSMB)

  • Personal statement and photo

  • Letters of recommendation, now through the AAMC Letter Writer Portal

  • MSPE and medical school transcript

  • Program signals, where your specialty uses them

A 12-month plan

Months before: 12–9 · Goals: Step 2 CK, ECFMG and OET, CV, research and USCE

Months before: 9–6 · Goals: Ask for letters, draft your personal statement, first list of programs

Months before: 6–3 · Goals: ERAS token, documents, photo, signals, budget

Months before: 3–0 · Goals: Submit, interviews, professional follow-up, NRMP registration

Months before: Match Week · Goals: Results, SOAP if needed, next steps

Red flags: anyone who guarantees an interview, a letter or a Match; paid experiences with no written contract; and invented experiences or publications. Programs check.

When interviews come, practice with the Project IMG interview simulator on doctorsebas.com and our live interview prep Discord.

Dr. Sebas tip: Rank programs by where you truly want to train, not by where you think you will match. The algorithm works in your favor when your list is honest.

Finding research you can contribute to

Start with a question you want to understand and a mentor whose work fits it. Offer a concrete, realistic contribution: a literature review, supervised data work, an approved project task or help organizing a study. State your current skills honestly, ask how supervision works and clarify the time commitment. Research is not a substitute for clinical eligibility or a guarantee of an interview.

Keep a record of your actual contribution, the project’s approval requirements, team responsibilities, deliverables and publication status. Do not call a submitted paper published or assume authorship before the team discusses it.

Prepare for interviews with your own stories

Build a small story bank covering teamwork, feedback, a challenge, patient communication and why your specialty fits you. Practice speaking aloud, listen for vague answers and refine one response at a time. Use the simulator as rehearsal, then discuss your application with a trusted mentor.

Application services differ

Check the service used by your specialty and each program. ERAS is an application service, while the NRMP operates a matching process; registering with one does not automatically register you with the other. Some specialties use other application or matching systems. Keep those accounts and deadlines separate.

Does an observership usually include hands-on patient care?

No, it is usually observation only — Confirm in writing what you will be allowed to do.

How do letter writers submit letters for the 2027 ERAS season?

Through the AAMC Letter Writer Portal — AAMC moved letters to the Letter Writer Portal starting with ERAS 2027.

7. Visa sponsorship and medical licensure planning

Start with the program and the official sponsor

Visa sponsorship, permission to work, a training permit, an unrestricted medical license and specialty board certification are different requirements. A program’s willingness to interview an applicant does not establish that it can sponsor that applicant’s training.

Questions to ask before you apply

Ask which immigration categories the institution supports for the current cycle, which office handles sponsorship, what examination or document deadlines it sets, and whether the policy applies to your track. Request the current written instructions rather than relying on an old program list.

J-1 physician sponsorship

Intealth’s Exchange Visitor Sponsorship Program administers J-1 sponsorship for eligible foreign national physicians in U.S. clinical training. Read its current application and training rules with your institution’s responsible office. Ask about the documents required for your circumstances, renewal timing, permitted activities and obligations after training.

Employer-sponsored options

Some institutions consider an H-1B or another appropriate category. Availability and eligibility depend on the petition, your circumstances, institutional policy and applicable law. Confirm the plan with the institution’s immigration office or qualified counsel. Do not base your application budget on a generalized social-media claim about fees or exemptions.

Licensure after the exams

Passing the USMLE sequence does not itself issue a medical license. Each state medical board sets its application, credential, training and examination requirements. Read the board’s current instructions for the state where you intend to train or practice, including rules that may depend on your educational background.

Your planning sheet

Record: institution and track; sponsorship office; current policy source and date; required documents; examination timing; training-permit requirements; renewal steps; and questions awaiting written clarification. Review it before committing to an offer or an application expense.

This section explains how to research the process. It does not determine personal immigration or licensure eligibility.

8. Study toolkit and common IMG questions

Build a plan from your available time

Choose the exam you are preparing for, a realistic number of weeks and the hours you can consistently protect. Set a baseline assessment, schedule question blocks and error review, then reserve time to reassess before your intended test date. Your plan should change when the results show a different need.

Use all six planning tools

Study plan: list the weekly question and review tasks. Budget: separate official fees from your own estimates and update them before paying. Self-assessment tracker: record the date, assessment, result and next action. Error log: classify the reason for a mistake. Biostatistics practice: explain the meaning of each measure rather than memorizing formulas alone. Registration checklist: track identity, eligibility, permit, appointment and travel.

A useful error log

For each missed question, label the main problem: knowledge, reasoning, reading, timing or language. Write one short rule, test it on a new question and return to it later. Copying a long explanation creates notes; applying the rule shows learning.

How to review your self-assessments

Compare assessments over time and note the circumstances: interrupted study, fatigue, a changed test format or an unfamiliar question style. A single practice result cannot promise an examination outcome. Use the official assessment interpretation and a mentor’s input when deciding whether to test.

Common USMLE and IMG questions

Do I have to take Step 1 before Step 2 CK?

No. Once you are eligible you can take them in either order. Most people take Step 1 first because it builds the base for Step 2 CK.

Is Step 1 still important if it is Pass/Fail?

Yes. Passing Step 1 is part of ECFMG’s medical science examination requirement. Prepare for a confident pass and use the foundation to support later clinical learning.

Where do I register now?

In the FSMB USMLE portal, for all three Steps. ECFMG (Intealth) still handles your eligibility, credentials, Pathway and certification through MyIntealth.

Can I take Step 3 outside the United States?

No. Step 3 is given only in the U.S. and its territories, such as Puerto Rico and Guam.

Do I need Step 3 to match?

Step 3 is not a universal prerequisite for matching. Check your own eligibility and each program’s requirements before deciding when to take it.

Does passing Step 3 give me a license?

No. Each state has its own requirements, including residency training.

What OET scores does ECFMG need?

350 in Listening, Reading and Speaking and 300 in Writing, all in the same sitting, from a test taken on or after January 1, 2025.

Can I take OET at home?

For ECFMG, only if your country has no physical OET venue. Otherwise take it on paper or computer at a venue.

Which Pathway is mine?

Use the current ECFMG eligibility page for all six Pathways. Licensed applicants may qualify for Pathway 1; other routes depend on accepted OSCEs, school criteria or supervised clinical encounter evaluations. Prior Step 2 CS failures have a specific Pathway 6 rule. Do not select a route from a short summary alone.

How many times can I take a Step?

At most three times in 12 months. A fourth attempt needs 12 months since the first and 6 months since the last. Four or more attempts at any one Step without passing (incomplete attempts count) make you ineligible for every Step.

Can I retake a Step I passed to improve my score?

You generally cannot retake a passed Step to raise your score. The USMLE policy describes limited exceptions for time limits imposed by recognized authorities.

Is there a time limit to finish everything?

ECFMG applies a seven-year window to its examination requirements, including Pathway acceptance. Pathway expiration and state licensing-board time limits are separate issues; track them separately.

How long do scores take?

Usually within four weeks of your test date. Allow up to eight weeks.

Is an observership hands-on?

Usually not. It is observation. Get in writing what you will be allowed to do, and don’t assume a letter of recommendation comes with it.

Which resources should I buy?

Start with the free official materials on usmle.org and plan the NBME self-assessments. Add one main question bank. Add other resources only to fix a specific weakness your data shows.

What is the WFME?

The World Federation for Medical Education. It recognizes accrediting agencies that meet international standards. It does not accredit individual schools. See the Accreditation stop and the agency finder.

If my school does not meet the Recognized Accreditation Policy, can I still take the USMLE?

Yes. Intealth states the policy does not affect USMLE eligibility or eligibility for ECFMG Certification. It does matter for Pathway 3, and Intealth says the policy may evolve.

How do I know if my school meets the accreditation policy?

Search your school in WDOMS, open the Sponsor Notes tab and look for the notation below the ECFMG Sponsor Note.

Is there a WFME-recognized agency in Latin America?

Yes. Argentina (CONEAU), Brazil (SAEME), Chile (CNA-Chile), Colombia (CNA), and Mexico, Costa Rica, Panama, El Salvador and Guatemala (COMAEM). The Caribbean has CAAM-HP and ACCM. Check the World atlas for every country.

What is on each Step?

Each Step stop has the official content outline from usmle.org as interactive bars, by system, discipline and physician task.

Can I trust dates I see on social media?

No. Use the official sites: USMLE, FSMB, NBME, Intealth/ECFMG, OET, Prometric, AAMC and NRMP.

Choose a practice tool for your next task

9. Country reference: test centers, accreditation and the 213-record atlas

World atlas: where to test and how your school is accredited

Every country and territory in one place. Switch the layer to see where you can sit Step 1 and Step 2 CK, or which countries have a WFME-recognized accrediting agency. Tap any country for its full card.

How to read the test-center layer

  • Reported means the country appeared in 2026 Prometric USMLE listings. Centers open, close and fill up, so confirm a real seat in the Prometric locator before you plan anything.

  • Step 3 is only given in the U.S. and its territories, wherever you live.

  • Testing Step 1 or Step 2 CK outside the U.S. and Canada adds the international surcharge.

  • OET runs at venues in many countries on paper or computer. ECFMG accepts OET@Home only if your country has no venue.

How to read the accreditation layer

It shows whether an accrediting agency that works in that country is recognized by the World Federation for Medical Education (WFME). That is about the agency, not your school. The next stop explains why it matters and how to check your own school.

Dr. Sebas tip: Testing abroad? Book the seat first, then the trip. Arrive the day before, find the building, and time the route at the same hour as your appointment.

Build a country-specific plan

Check the current Prometric locator for the exact examination, country and dates. A country appearing in a past list does not establish that an appointment is available today. An accreditation agency’s presence in a country does not determine your individual school’s ECFMG eligibility.

Country and accreditation research checklist

Record your country of residence, intended test country, travel-document requirements, test-center appointment, total checkout fee, transport and contingency plan. Separately record the medical school’s full name, campus, graduation year, ECFMG Sponsor Note and the school’s accrediting agency.

Use the complete interactive country directory

The original roadmap contains a country-and-territory atlas with a test-center layer, WFME agency information and a country search. Use it to begin your research, then confirm current availability with Prometric, school eligibility with WDOMS and recognition status with WFME.

Country and territory reference: 213 roadmap records

This is the country reference contained in the interactive roadmap, published here as readable text on October 19, 2026. It is a planning snapshot, not a live appointment inventory or a determination of school eligibility. “Reported” means listed in the roadmap’s 2026 test-center layer. Recheck the official Prometric locator for your examination and dates, WFME for agency recognition, and WDOMS for the actual school’s Sponsor Note.

The agency information below describes the roadmap record for an agency operating in a country. It does not mean every school in that country is accredited by that agency or is ECFMG eligible. A recognition end date is not your medical school’s accreditation expiry date.
