70+ Hospital Rotations, USCE & Observership Opportunities for IMGs | 2026–2027
Updated: 4 days ago
Hello guys ❤ I put this guide together because finding a U.S. rotation should not mean spending your whole night chasing old links, confusing prices and unanswered emails. I wish I had a clearer starting point when I was a student. Let’s make your next step a little easier 🙌

UPDATED OCTOBER 2026 ❤ Explore the opportunities below and choose what fits your stage of training, specialty and budget. Before applying, confirm eligibility, available dates and total costs directly with the program.
Let’s find your next rotation ❤
Below you’ll find each institution or program, its location, who can apply, requirements, costs when available, and the website or contact to get started. Some universities offer separate opportunities in different departments, so read each entry carefully.
Choose the experience that matches YOUR status
CURRENT STUDENT → Start with visiting electives/clerkships. Many require final-year enrollment, completed core rotations, home-school approval, health clearance and malpractice coverage. VSLO access depends on your school and host.
MEDICAL GRADUATE → Look for graduate observerships or an explicitly approved graduate training route. A student elective usually cannot be taken after graduation.
OBSERVERSHIP → Observation is different from hands-on patient care. Confirm the permitted activities, teaching, evaluation and letter policy. A certificate or hospital name does not establish that a residency program accepts the experience as USCE.
EXTERNSHIP → Providers use this word differently. Ask for the actual duties and institutional approval, not just the marketing label. Your professional status and immigration permission also matter.
RESEARCH / VIRTUAL EXPERIENCE → Useful learning can still be valuable, but do not describe it as in-person clinical work. Ask target residency programs what they accept.
Before you apply or pay
SAVE THIS CHECKLIST ❤ 1. Am I eligible as a student or graduate? 2. What will I actually do? 3. Who supervises me, and at which exact site? 4. Is a letter possible, who writes it and on which letterhead? 5. What is the TOTAL cost and refund policy? Get those answers in writing.
Budget for the rotation/application fees, health and malpractice insurance, required screening and documents, housing, meals, transportation and travel. Keep an emergency cushion. An expensive rotation is not automatically the right rotation for you, and no experience guarantees a residency interview or Match.
Do not buy flights or non-refundable housing before written acceptance and clearance. Use the host’s instructions for your particular role and immigration status; rules differ between institutions. An invitation letter does not guarantee visa issuance or authorize duties.

A simple email to start the conversation
Subject: [Specialty] observership/elective inquiry — [Month, Year] Hello [Coordinator/Program Team], My name is [Name]. I am a [medical student in my final year / medical graduate] from [School, Country], interested in [Specialty]. Could you please confirm whether I am eligible for your program, the available dates, application process, permitted activities, complete costs and evaluation/letter policy? My preferred dates are [Dates]. Thank you for your time and guidance. Kind regards, [Name]
Personalize it! Explain why this particular program interests you, keep the first inquiry short, and follow the official application process. If the page says not to contact faculty, respect that instruction. Send documents only through the program's approved submission route.
Make the experience count ❤
Prepare before you arrive: learn the team’s workflow, review common conditions in your specialty, show up on time, protect patient privacy and ask thoughtful questions. Learn from nurses, residents, attending physicians and every member of the team. Your goal is to become a better colleague and physician.
If a letter is permitted, give the writer a clear picture of your actual dates, activities and goals, and ask early enough for a thoughtful assessment. A genuine, specific letter matters more than a famous name on a letter describing work you did not do.
ONE STEP AT A TIME ❤ Choose five–ten routes that match your status, budget and specialty. Track the official source, date checked, written eligibility confirmation, total costs and next action. You do not need to apply everywhere to move forward.
Please save this guide and share it with a friend who needs a clearer starting point. If you find a changed program policy or a new official opportunity, send the source so we can help the next IMG too. Love you all! I want you to succeed and build a journey that is truly yours ❤

Wow, this is an incredible resource for IMGs looking to get rotations in the US! Thank you so much for putting all this information together.
Including Ivy League universities is awesome - those can be really competitive and having the specific requirements and contact info is super helpful. It's great that you've included fees too, since that can be a major factor for many students.
I'm really curious about your comments at the end. Do you have any insider tips or insights that aren't commonly known? Maybe about how to make an application stand out or the best times to apply?
This kind of comprehensive guide can be a game-changer for IMGs navigating the US medical system. Do you have…
Kindness level🔝 ,so humble of u.The line hit so deep to my heart " I don't want student money " . You are a symbol of mankind ,bow down to u sir🙏. Really appreciate this initiative.
What are my options as an international medical graduate from India. I’m a green card holder
Icahn School of Medicine is not accepting students from schools that aren't an active member of VSLO so students from Argentina can not apply!
Hi Sebas, I want to tell you that I have contacted recently with University of South Carolina and they told me that they don't have enough space for education for IMS.
Best regards,
